Two things to try, using the latest 3.6.0.48
and if that alone does not fix the problem then try again but this time uncheck
- Use 2 pass encoding
- Use best video quality settings

You can try with 4.0.0.13
you can try on your 2nd conversion with the menu template "no menu"
and if neither work work we can connect to your machine to see what could be the problem. I've sent you an email.
