DVD Converter Problem

When I am interested in a program, I try to test it thoroughly to see how it performs and to check for possible errors in its operation. I've been testing this one for a few days and now I've just checked (2 times it has happened to me) that I'm converting (2 passes) a DVD with 5 episodes of a series that the 1st pass does well in all episodes, but the 2nd pass only makes them in the first 4 episodes, the 5th I have to do individually. What is what can happen and if there is any solution in this regard?
